I have received some forms for pre-existing conditions from my insurance. What do I do with them?

A. When you receive forms requesting information regarding pre-existing conditions, you must fill out and submit the forms to the insurance in a timely manner. This ensures timely processing of your claim. Any delay in submitting the forms may result in the insurance company denying your claim making you liable for the amount.
